Evaluation and predictive value of P16 and Ki-67 protein expression in HPV infection after cervical lesions sur-gery

Abstract:Objective To evaluate and predict the therapeutic efficacy of loop electrosurgical excision procedure(LEEP)combined with anti-human papillomavirus(HPV)protein dressing in patients with HPV-associated cervical lesions based on the expression levels of tumor suppressor gene P16 and proliferation marker Ki-67.Methods A total of 300 patients with HPV-associated cervical lesions treated at the First Affiliated Hospital of Xinxiang Medical University from January 2022 to January 2024 were enrolled.All patients underwent LEEP combined with topical anti-HPV protein dressing therapy and were followed up for 12 months.Based on their prognosis,they were divided into a favorable progno-sis group(n=249)and a poor prognosis group(n=51).The expression levels of P16 and Ki-67 before and 60 days af-ter treatment were compared.The predictive value of P16 and Ki-67 for poor prognosis was assessed using ROC curves,and logistic regression was conducted to identify risk factors for poor outcomes.Results At 60 days post-treatment,the overall positive rates of P16 and Ki-67 were significantly lower than at admission(P<0.05).The favorable and poor prognosis groups both showed significant reductions in P1 6 and Ki-67 positivity post-treatment(P<0.05),but levels remained significantly higher in the poor prognosis group(P<0.05).ROC analysis revealed that the combined prediction model of P16 and Ki-67 had an area under the curve(AUC)of 0.843(95%CI:0.784-0.902),which was superior to either marker alone(P<0.05).Logistic regression indicated that positive expression of P1 6 and Ki-67 were inde-pendent risk factors for poor prognosis in patients with HPV-related cervical lesions(P<0.05).Conclusion LEEP combined with anti-HPV protein dressing can reduce P16 and Ki-67 positivity in HPV-associated cervical lesions.Positive expression of P16 and Ki-67 are associated with poor prognosis,and their combined use enhances the predictive accuracy for adverse outcomes.
