Clinical significance of peripheral blood S100A4 and S100A8 detection during acute exacerbation of mycoplasma pneumoniae-induced asthma in children

Abstract:Objective To investigate the relationship between peripheral blood levels of calcium-binding protein A4(S100A4)and A8(S100A8)and the severity and control of asthma in children with acute exacerbations induced by Mycoplasma pneumoniae(MP)infection.Methods A total of 125 children with MP-induced acute asthma exacerba-tions(asthma group),125 children with MP infection without asthma(MP group),and 125 healthy children(control group)treated from June 2020 to December 2023 were included.The asthma group was further divided into a mild group(68 cases)and a severe group(57 cases)based on the severity of acute exacerbation.Peripheral blood S100A4 and S100A8 levels were measured before treatment,and asthma control was evaluated post-treatment.Based on control sta-tus,the asthma group was further divided into well-controlled(79 cases)and poorly controlled(46 cases)subgroups.Multivariate logistic regression was used to analyze factors influencing poor asthma control.ROC curve analysis was per-formed to assess the predictive value of S100A4 and S100A8.Results Peripheral blood levels of S100A4 and S100A8 in the asthma group[(38.26±7.43)pg/mL and(196.35±42.82)pg/mL,respectively]were significantly higher than those in the MP group[(29.24±5.09)pg/mL and(138.42±30.11)pg/mL]and control group[(19.65±2.57)pg/mL and(72.35±16.54)pg/mL](P<0.05).S100A4 and S100A8 levels in the severe subgroup were significantly higher than those in the mild subgroup[(36.25±3.41)pg/mL and(198.40±15.73)pg/mL vs.(33.16±2.03)pg/mL and(168.44±9.67)pg/mL](P<0.05).Levels were also higher in the poorly controlled group compared to the well-controlled group[(39.71±1.16)pg/mL and(202.28±8.42)pg/mL vs.(34.06±1.24)pg/mL and(163.15±14.08)pg/mL](P<0.05).Elevated eosinophil count,high S100A4,and high S100A8 were identified as risk fac-tors for poor asthma control(P<0.05).The AUCs for S100A4 and S100A8 in predicting poor control were 0.763 and 0.794,respectively,and the combined AUC was 0.905,which was significantly higher than either marker alone(P<0.05).Conclusion Elevated peripheral blood S100A4 and S100A8 levels during acute exacerbation in MP-induced asthma in children are associated with more severe disease and poor control.Combined detection of S100A4 and S100A8 has a high predictive value for asthma control outcomes.
