Differentiation of intrahepatic mass-forming cholangiocarcinoma and hepatocellular carcinoma using CT histo-gram texture analysis

Abstract:Objective To evaluate the diagnostic value of CT histogram texture features in differentiating intrahe-patic mass-forming cholangiocarcinoma(IMCC)from hepatocellular carcinoma(HCC).Methods A retrospective analysis was conducted on contrast-enhanced CT images from 34 patients with pathologically confirmed IMCC and 31 pa-tients with HCC.Texture analysis software was used to extract first-order grayscale histogram features,including mean,variance,skewness,kurtosis,and percentiles(1%,10%,50%,90%,99%)across arterial,portal venous,and equi-librium phases.Statistical analysis was performed to assess the diagnostic performance of these features.Results Among individual texture features,the 10th percentile(perc10)during the portal venous phase showed the highest diagnostic val-ue with an AUC of 0.771,sensitivity of 79.4%,and specificity of 71.0%.Combined texture parameters in the equilibri-um phase yielded the highest overall diagnostic performance with an AUC of 0.884,sensitivity of 97.1%,and specificity of 77.4%.Conclusion CT-based histogram texture analysis provides valuable information for differentiating IMCC from HCC,with equilibrium-phase texture features showing the greatest diagnostic efficacy.
