Effect of repetitive transcranial magnetic stimulation combined with scalp acupuncture based on the central-pe-ripheral-central closed-loop rehabilitation theory on post-stroke cognitive impairment

Abstract:Objective To observe the effect of repetitive transcranial magnetic stimulation(rTMS)combined with scalp acupuncture based on the central-peripheral-central closed-loop rehabilitation theory on post-stroke cogni-tive impairment(PSCI)in patients with first-onset stroke.Methods A total of 104 patients with first-onset PSCI be-tween March 2020 and March 2024 were randomly divided into two groups(n=52 each).The control group received rT-MS combined with scalp acupuncture,while the observation group received cognitive rehabilitation training in addition to rTMS and scalp acupuncture under the guidance of the"central-peripheral-central"closed-loop rehabilitation theory.Cognitive function and daily memory were evaluated using the Mini-Mental State Examination(MMSE),Montreal Cogni-tive Assessment(MoCA),and Rivermead Behavioral Memory Test(RBMT).Activities of daily living were assessed u-sing the Functional Independence Measure(FIM)and the Modified Barthel Index(MBI).Serum levels of interleukin-6(IL-6)and brain-derived neurotrophic factor(BDNF)were also measured.Adverse reactions were recorded and com-pared between groups.Results After treatment,both groups showed significant improvements in MMSE,MoCA,RBMT,FIM,and MBI scores,as well as increased serum BDNF levels(P<0.05),with greater improvements in the observation group compared to the control group(P<0.05).Serum IL-6 levels decreased in both groups after treatment,with the observation group showing significantly lower levels than the control group(P<0.05).The incidence of adverse reactions did not differ significantly between groups(P>0.05).Conclusion Cognitive rehabilitation training combined with rTMS and scalp acupuncture,guided by the central-peripheral-central closed-loop rehabilitation theory,can effectively im-prove cognitive function and memory in first-onset PSCI patients,enhance their quality of life,and reduce inflammatory responses,with good safety and tolerability.
