Changes in serum soluble myeloid cells trigger receptor-1 and suppressor of cytokine signaling 3 levels in chil-dren with recurrent respiratory tract infections and their clinical significance

Abstract:Objective To investigate the changes in serum soluble myeloid cells trigger receptor-1(sTREM-1)and suppressor of cytokine signaling 3(SOCS3)levels in children with recurrent respiratory tract infections(rRTI),and to evaluate their clinical value in predicting rRTI.Methods A total of 118 children diagnosed with rRTI and admit-ted to the Department of Pediatrics,Guangzhou Red Cross Hospital between January 2022 and July 2024 were enrolled as the rRTI group.Based on disease severity,they were further divided into mild(n=47),moderate(n=39),and severe(n=32)subgroups.Additionally,80 healthy children undergoing routine physical examinations during the same period were included as the control group.General and clinical data were compared between the rRTI and control groups.Serum levels of sTREM-1 and SOCS3 were analyzed across severity subgroups.Logistic regression was performed to identify in-dependent risk factors for rRTI,and receiver operating characteristic(ROC)curve analysis was used to assess the predic-tive value of sTREM-1 and SOCS3.Results Significant differences were observed between the rRTI and control groups in terms of annual antibiotic use,daily outdoor activity time,interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),immunoglobulin(Ig)A,IgG,IgM,sTREM-1,and SOCS3 levels(P<0.05).Serum sTREM-1 and SOCS3 levels increased progressively with rRTI severity(P<0.05),with pairwise comparisons among severity subgroups showing significant differences(P<0.05).Multivariate logistic regression identified annual antibiotic use,TNF-α,sTREM-1,and SOCS3 as independent risk factors for rRTI,while daily outdoor activity time,IgA,IgG,and IgM were protective fac-tors.The combined predictive value of sTREM-1 and SOCS3 yielded an AUC of 0.850(95%CI:0.748-0.913),which was significantly higher than the AUCs for sTREM-1 or SOCS3 alone(Z=3.647,3.015;P=0.004,0.009,re-spectively).Conclusion Serum sTREM-1 and SOCS3 levels are significantly elevated in children with rRTI and serve as independent risk factors for disease occurrence.The combined use of sTREM-1 and SOCS3 has high clinical value in predicting rRTI.
