Analysis of Risk Factors for Renal Injury Caused by Two Hepatoprotective Drugs During Anti-Tuberculosis Treatment in Newly Diagnosed Drug-Sensitive Patients

Abstract:Objective To systematically analyze the risk factors for renal injury caused by two hepatoprotective drugs during anti-tuberculosis treatment in newly diagnosed drug-sensitive tuberculosis patients.Methods A retro-spective analysis was conducted on clinical data from 215 newly diagnosed drug-sensitive tuberculosis patients treated be-tween September 2019 and September 2022.Patients were grouped based on the occurrence of renal injury during treat-ment.Univariate analysis was performed on general demographics,disease-related factors,and baseline liver and kidney function parameters between the renal injury and non-injury groups.Variables with statistical significance were further analyzed using binary logistic regression to identify independent risk factors.Results Among the 215 patients,36 experi-enced renal injury and were assigned to the renal injury group,while 179 without renal injury were assigned to the non-injury group.No significant differences were observed between the two groups regarding baseline characteristics,site of tu-berculosis,concomitant medications,or pre-treatment ALT,AST,DBIL,and TBIL levels(P>0.05).However,sig-nificant differences were found in age and type of hepatoprotective drug used(P<0.05).The renal injury group had higher baseline serum creatinine(Scr)and blood urea nitrogen(BUN)levels and lower creatinine clearance rate(CCr)compared to the non-injury group(P<0.05).Logistic regression analysis revealed that the use of tiopronin was an inde-pendent risk factor for renal injury,while a normal baseline CCr was a protective factor.Conclusion During hepatopro-tective treatment in patients undergoing anti-tuberculosis therapy,tiopronin use and pre-treatment renal function are po-tential risk factors for developing renal injury.A normal pre-treatment CCr serves as a protective factor.Therefore,for patients with low baseline CCr,careful consideration should be given when selecting hepatoprotective agents that may affect renal function.
