Association between hyperparathyroidism and all-cause mortality in maintenance hemodialysis patients with ure-mia

Abstract:Objective To investigate the association between hyperparathyroidism and all-cause mortality in pa-tients with uremia undergoing maintenance hemodialysis.Methods A retrospective study was conducted on 95 uremic patients receiving maintenance hemodialysis in our hospital from January 2021 to January 2023.Patients were divided into a hyperparathyroidism group(n=51)and a non-hyperparathyroidism group(n=44)based on parathyroid hormone(PTH)levels.Clinical characteristics,serum calcium and phosphate levels,and dialysis-related parameters were col-lected.One-year survival differences between groups were analyzed using Kaplan-Meier survival curves and the Log-rank test.Cox proportional hazards regression analysis was used to identify independent risk factors for one-year all-cause mortality.Results There were no statistically significant differences between the two groups in terms of age,gen-der,thrombosis,diabetes,and coronary artery disease(P>0.05).The hyperparathyroidism group had significantly higher levels of serum calcium and phosphate compared to the non-hyperparathyroidism group(P<0.05).Dialysis du-ration,frequency,and adequacy indicators showed no significant differences between groups(P>0.05).Kaplan-Meier analysis revealed a one-year survival rate of 58.82%(30/51)in the hyperparathyroidism group and 82.93%(34/41)in the non-hyperparathyroidism group(Log-rank x2=7.122,P=0.008).Cox regression analysis identified hyperpar-athyroidism,elevated serum calcium,and phosphate as independent risk factors for all-cause mortality within one year(P<0.05).Conclusion Uremic patients with hyperparathyroidism exhibit elevated serum calcium and phosphate levels and higher mortality.Hyperparathyroidism is significantly associated with an increased risk of all-cause mortality in ma-intenance hemodialysis patients.
