Application of comfort-oriented care units in the post-anesthesia care unit for elderly patients after hip replace-ment surgery

Abstract:Objective To explore the role and effectiveness of comfort-oriented care unit management in the post-anesthesia care unit(PACU)for elderly patients undergoing hip replacement,and to provide reference for perioper-ative management in this population.Methods A total of 60 elderly patients who underwent hip replacement under gen-eral anesthesia and entered PACU observation at Renmin Hospital of Wuhan University from March to August 2024 were randomly divided into a control group and an observation group.After admission,all patients were assessed using the Vis-ual Analogue Scale(VAS)for pain and the Sedation-Agitation Scale(SAS)for cooperation.For patients in the observa-tion group with a VAS score ≥4,fascia iliaca compartment block(FICB)was performed under ultrasound guidance.SAS and VAS scores were recorded at 5,15,30,and 60 minutes after nerve block,along with the incidence of delirium within 24 hours postoperatively,PACU stay duration,and patient comfort levels.Results Compared to the control group,pa-tients in the observation group had significantly lower SAS scores at 15,30,and 60 minutes after nerve block(6.23±0.63 vs.5.27±0.83;6.50±0.63 vs.5.10±0.80;6.30±0.70 vs.4.60±0.77),and lower VAS scores(6.80±0.89 vs.5.07±1.01;6.50±0.63 vs.4.10±0.76;6.60±0.72 vs.4.33±0.61).The 24-hour postoperative deliri-um incidence was also reduced(26.67%vs.6.67%),PACU stay was shortened[(103.93±17.22)min vs.(82.33±7.04)min],and patient comfort was significantly improved(56.67%vs.93.33%).Conclusion Implementing comfort-oriented care in PACU for elderly patients undergoing hip replacement can effectively relieve postoperative pain,reduce delirium incidence,shorten PACU stay,and enhance patient comfort.This approach is worthy of clinical promotion.
