Expression levels of midkine in sepsis patients and its correlation with ischemic-hypoxic injury and prognosis

Abstract：Objective To analyze the expression levels of midkine(MK)in patients with sepsis and to investi-gate the correlation between MK with ischemic-hypoxic injury and prognosis in patients with sepsis.Methods A total of 98 sepsis patients treated in the intensive care unit(ICU)from January 2023 to July 2024 were enrolled in this study and followed up for 28 days,with 4 cases lost to follow-up.Patients were divided into a survival group(n=64)and a mortality group(n=30)based on their prognosis.Clinical data were collected,and Pearson correlation analysis was used to explore the relationship between ischemic-hypoxic injury markers and MK levels.A multivariate logistic regression model was applied to identify prognostic factors in sepsis patients and to evaluate the predictive performance of MK in prog-nosis.Results The levels of MK,heart rate,mean arterial pressure(MAP),central venous pressure(CVP),oxygen delivery(DO2),oxygen consumption(VO2),oxygen extraction ratio(O2ER),and lactate differed significantly between the two groups(P＜0.05).Pearson correlation analysis showed that MK was negatively correlated with DO2(r=-0.306,P=0.000)and positively correlated with VO2(r=0.309,P=0.001),O2ER(r=0.380,P=0.001),and lactate(r=0.523,P=0.000).Univariate logistic regression analysis revealed that MK(OR=213.983,P=0.000),MAP(OR=0.930,P=0.000),CVP(OR=0.294,P=0.007),DO2(OR=0.973,P=0.000),O2ER(OR=1.477,P=0.000),and lactate(OR=24.556,P=0.000)were significant prognostic factors in sepsis patients.Multivari-ate logistic regression analysis identified MK(OR=175.258,P=0.008)as an independent risk factor for sepsis prognosis.Receiver operating characteristic(ROC)curve analysis showed that MK had high predictive efficacy for sepsis prognosis(AUC=0.821,95％CI:0.727-0.893),with a sensitivity of76.67％and specificity of 77.78％.Conclusion MK is significantly correlated with VO2,DO2,O2ER,and lactate,and it serves as an independent prognostic risk factor in sep-sis patients.MK can be used as a crucial biomarker for predicting the prognosis of sepsis patients.
