A comparative study on the efficacy of different incisions in microscopic treatment of left-sided varicocele

Abstract:Objective To compare the clinical outcomes of microscopic varicocelectomy through different inci-sions in the treatment of left-sided varicocele.Methods The clinical data of 60 adult patients with moderate or severe left-sided varicocele who underwent microscopic varicocelectomy were retrospectively analyzed.The patients were divided into two groups:one group(30 cases)underwent surgery via an inguinal incision,and the other group(30 cases)through a scrotal root incision.We compared various surgical parameters between the two groups,including incision length,blood loss,spermatic cord dissection time,varicocele ligation time,total surgical time,number of varicoceles,incidence of scrotal edema and wound infection,postoperative pain scores,and sperm quality.Results The scrotal root incision group required significantly less time for spermatic cord dissection,experienced less blood loss,and had shorter incisions com-pared to the inguinal group.The incidence of scrotal edema and wound infection was lower in the scrotal root incision group(P<0.05).Additionally,postoperative pain scores were significantly lower in this group(P<0.05).There were no significant differences in total surgical time and sperm quality between the two groups(P>0.05).The number of vari-coceles was higher in the scrotal root incision group compared to the inguinal group(P<0.05).Conclusion The scrotal root incision offers several advantages,including less blood loss,smaller surgical incisions,lower rates of scrotal edema and wound infection,and reduced postoperative pain.It does not increase total surgical time and effectively improves sperm quality postoperatively.The scrotal root incision also provides easier access to the spermatic cord,making it an ide-al choice for novice surgeons.
