The neurological manifestations in 32 patients with primary Sjögren's syndrome

Abstract:Objective To investigate the clinical features of neurological involvement in primary Sjögren's syndrome(pSS).Methods The clinical data of 32 pSS patients with neurological involvement from March 2013 to Jan-uary 2022 were retrospectively reviewed.Clinical data were compared between patients with demyelinating disease and vas-cular disease.Results The prevalence of neurological involvement in pSS patients was 11.1%(32/287),while 56.3%(18/32)as the initial symptom of pSS.There were a variety of neuropsychiatric presentations in pSS.The incidence of central neuropathy,peripheral neuropathy and combination of the central neuropathy with peripheral neuropathy were 84.4%(27/32),6.3%(2/32)and 6.3%(2/32),respectively.Neuromuscular junction lesion was found in 1 patient(3.1%).Thirteen patients were diagnosed with demyelinating disease,13 had vascular disease,3 had infectious dis-ease,2 patients had tension headache and 1 patient was diagnosed with myasthenia gravis.Compared with those patients with vascular disease,lymphocytes and serum IgM level were significantly higher in patients with demyelinating disease[1.98(1.39,2.77)× 109·L-1 vs.1.30(1.07,1.77)× 109·L-1,P=0.005;1.31(1.12,1.88)g/L vs.0.98(0.74,1.18)g/L,P=0.009].Conclusion The prevalence of neurological involvement in pSS is not low.The clinical mani-festations are diverse and complex.Demyelinating diseases and vascular diseases are common.The pathogenesis is un-known.Lymphocyte activation and immune system over-stimulation may play roles in neurological involvement in pSS.
