Characteristic analysis of surgical complications in a tertiary hospital

Abstract：Objective To analyze the complications of patients undergoing elective surgery,to explore the char-acteristics and influencing factors of surgical complications,and to provide data reference for continuous improvement of medical quality.Methods The medical records of surgical patients in one of Guangzhou Hospital from January 1,2018 to December 31,2022 were collected,and the patients with complications were described,including basic information,types of complications,incidence rates over the years,disease distribution,etc.Binary logistic regression was used to an-alyze the influencing factors of surgical complications.Results Among patients with surgical complications,males out-numbered females,with the main age group being 50-69 years.About 60.52％underwent grade Ⅳ surgery,and 24.97％underwent minimally invasive surgery.The proportion of patients hospitalized for more than 30 days was 32.06％.Patient gender,age,length of hospital stay,discharge method,and whether there was a readmission plan within 31 days were significantly correlated with the occurrence of surgical complications(P＜0.001).The incidence rates of surgical complications from 2018 to 2022 were 1.79％,1.42％,1.71％,1.31％,and 0.96％,respectively.The main types included pulmonary infections(1 560 cases),surgical wound dehiscence(495 cases),multiple complications(383 cases),bleeding or hematoma(259 cases),among which the incidence rates of bleeding or hematoma and thrombosis/embolism showed an increasing trend year by year.The main types of surgical complications occurred in patients with con-genital circulatory system malformations,ischemic heart disease,other types of heart disease,malignant tumors of the re-spiratory and thoracic organs,chronic rheumatic heart disease,and cerebrovascular diseases,among which the incidence rates of surgical complications in congenital circulatory system malformations,chronic rheumatic heart disease,and cere-brovascular diseases were 4.16％,4.60％,and 4.16％,respectively.Factors affecting the occurrence of complications in elective surgery patients mainly include gender,age ≥70 years,hospital stay ≥30 days,heart diseases,and readmis-sion plans within 31 days.Conclusion During the hospitalization of patients,attention should be focused on male,elder-ly,and long hospitalization,especially in surgeries related to heart diseases,to reduce the incidence of complications and continuously improve the level of medical quality and safety.
