Abstract:Objective To track the safety of single nucleic acid testing positive blood donors, thus to provide data support for the blood services. Methods A total of 2633 single NAT positive unpaid donor samples from Guangzhou blood center from January 2015 to June 2018 were analyzed. NAT HBV, HCV and HIV identification tests were performed for further evaluation, all blood donors were recalled for serologic screening program and nucleic acid detection. Results The single NAT positive rate and the NAT identification test positive rate of unpaid donors were 0.23% and 32.78%, respectively. Among the 665 single NAT positive blood donors recalled, there were 150 blood donors review showed unqualified items, including 21 cases of HBsAg positive, 3 cases of anti-HIV screening suspicious (send to the CDC and confirmed positive), and 110 cases were remain single NAT positive; so as their blood donation qualification had been shielded. The other 515 donors had passed all the screening programs, among whom 177 hadn′t donated blood again. Thirty-five reviewed blood donors were negative in NAT, but positive in retrospectively identify nucleic acid test. Twelve of them had one or more than one qualified blood donation record, but there were 11 blood donors appeared the single NAT positive results again in blood screening after unlock. Conclusion Nucleic acid detection technology can further reduce blood screening window, improve the safety of blood, but also causes a blood waste due to the false positive nucleic acid detection results. Some unlock screen blood donors show uncertain nucleic acid detection, the blood safety requires further confirmation. Blood services should perfectly shield blood donors and return strategy as soon as possible, to ensure blood safety as well as reduce blood loss.
