Effect of kidney function on high-density lipoprotein particle size in patients with coronary heart disease

Abstract:Objective To investigate the potential effect of kidney function on the size of high-density lipoprotein (HDL) particle in patients with coronary heart disease (CHD). Methods HDL-C and apolipoprotein A-I ratio (HDL-C/apoA-I) was used to represent HDL particle size. A total of 443 patients were enrolled. All of them received their first coronary angiography, and 336 of them were diagnosed as CHD, while the other 107 didn′t meet the criteria. HDL particle size was compared between various groups divided by eGFR and Gensini scores. The correlations between HDL particle sizes and the occurrence of CHD was analyzed. Results In CHD patients with kidney deficiency, HDL particles grew larger as kidney function declined, started from the eGFR of 60 mL/min·1.73 m2 (P<0.001). Smaller HDL particle was significantly associated with higher Gensini scores in CHD patients with normal kidney function (P=0.019), but the association was lost in CHD patients with decrease eGFR (P=0.653). Smaller HDL particle indicated a significantly higher risk of CHD with or without kidney deficiency. Conclusion Smaller HDL particle indicates a higher risk for CHD occurrence and severer coronary arteries stenosis in CHD patients with or without kidney deficiency. However, HDL particles shifted to larger sizes among CHD patients with kidney deficiency at the start of CKD 3, accompanied loss of the original association between HDL particle size and CHD occurrence.
