Clinical significance of insulin intensive therapy combined with plasma exchange on inflammatory indicators and prognosis of patients with hyperlipidemic pancreatitis

Abstract:Objective To investigate the effect of intensive insulin therapy combined with plasma exchange (PE) on inflammatory indicators and prognosis of patients with hyperlipidemic pancreatitis (HLP). Methods Thirty HLP patients were randomly divided into two groups. Group A received conventional treatment and PE treatment. Group B was treated with insulin intensive therapy besides conventional treatment and PE. Inflammatory indicators and prognosis were compared within the two groups. Results Compared with admission, APCHE Ⅱ scale and inflammatory indicators, including CRP, IL-6 and TNF-α decreased after treatment in both groups. Compared with Group A, no significant difference in inflammatory indicators, APCHE Ⅱ scale or Balthazar scale was revealed in Group B on Day 3; whereas inflammatory indicators, APCHE Ⅱ scale and length of ICU stay were significantly reduced in Group B on Day 5. Conclusion Compared with PE, intensive insulin therapy combined with PE can alleviate inflammatory response and promote prognosis in HLP patients.
