Cerebrovascular hemodynamic change in acute cerebral infarction patients with H type hypertension

Abstract:Objective To investigate the cerebrovascular hemodynamic changes in acute cerebral infarction (ACI)patients with H type hypertension.Methods A total of 331 ACI cases were selected,who were divided into four subgroups according to blood pressure condition and homocysteine(Hcy)level,Group A(H type hypertension),Group B(hypertension without hyperhomocysteine, HHcy), Group C(HHcy without hypertension)and Group D(control group,patients without hypertension or HHcy).Cerebral vascular hemodynamic index(CVHI)and other basic data were collected and analyzed by statistical software using analysis of covariance(ANCOVA).Results The CVHI total score was ranked from the lowest to the highest in the order Group A,B,C and D.It was significantly differed in Group A with the other 3 groups.Compared with Group A,significantly higher mean velocity of blood flow(Vmean),maximal velocity of blood flow(Vmax), minimal velocity of blood flow(Vmin), dilatation index(DI)and lower pulse wave velocity (Wv),characteristic impedance of cerebrovascular net(Zcv), and resistance of the vascular(Rv)were observed in Group B,C and D.Conclusion H type hypertension patients have less satisfactory cerebral hemodynamic condition in four aspects including large artery elasticity, small artery resistance and capillary shutting condition.Hypertension alone dominantly impairs small arteries and microcirculation while influences medium and large arteries when coexisting with HHcy.
