Non-invasive hemodynamic indicators in coronary CT angiography have predictive value for clinical outcomes of patients with unstable angina with intermediate lesions

Abstract:Objective To explore the predictive value of hemodynamic indicators such as computed tomography derived fractional flow reserve(CT-FFR),wall shear stress(WSS),and axial plaque stress(APS)based on coronary computed tomography angiography for major adverse cardiovascular events(MACEs)in patients with unstable angina with intermediate lesions.Methods A total of 92 patients with unstable angina complicated with intermediate lesions diagnosed by coronary computed tomography angiography and coronary angiography in the Department of Cardiology,the First Affiliated Hospital of Bengbu Medical University from January to December 2022 were selected for an average of 12 months of follow-up.LASSO regression model and multivariate Logistic regression analysis were used to screen the independent predictors of the prognosis of patients,and a line plot prediction model was constructed.ROC curve,calibration curve and decision curve analysis decision curve were used to verify the model.Results According to LASSO regression analysis and multivariate Logistic regression analysis,lipoprotein(a),CT-FFR,WSS and APS were selected as independent risk factors for the occurrence of MACEs in patients with unstable angina complicated with intermediate lesions.According to this,the area under the ROC curve of the nomogram was 0.987.The calibration curve and decision curve analysis suggested that the model had good fitting and high clinical utility.Conclusion The combination of lipoprotein(a)and non-invasive hemodynamic indicators such as CT-FFR,WSS,and APS in coronary computed tomography angiography has good predictive value for the occurrence of MACEs in patients with unstable angina complicated by intermediate lesions.
