The effect of clinical pharmacist medication guidance on the standardization of outpatient drug use in primary hospital

Abstract：To explore the effect of primary hospital clinical pharmacists to guide rational drug use of outpatient medication practices. Of outpatient prescriptions in our hospital from July 2011 to June 2013 (before intervention) and from July 2013 to June 2015 (after intervention), monthly randomly selected 500 copies, before intervention and after the extraction of the 12000 prescriptions were analyzed. Statistics of the national basic drug use proportion, number of irrational prescriptions, antibacterial drug usage, antibacterial drugs, prescription amount index before and after the intervention were studlied. After the rational intervention of pharmacist, our hospital outpatient national basic drug use rate from 44.20% (before the intervention )significantly increased to 67.50%(x2=7.87,P<0.05). The amount of basic drugs by 30.34±7.32 rose to 34.04±8.31 yuan (t=1.94,P<0.05), and the difference is statistically significant. Average amount of a single prescription by 74.53±10.89 yuan fell to 58.83±7.73 yuan(t=5.39,P<0.05); The number of irregular prescriptions decreased from 14.92%(before intervention)to 9.65%(χ2=7.32,P<0.05), The number of prescriptions was not appropriate for the number of prescriptions decreased from 8.80% (before the intervention ) to 6.35% .(χ2=6.33,P<0.05), Multiplexing medicine is not suitable by 5.64% decline to 3.61%(χ2=5.52,P<0. 05), route of administration is not suitable by 5.13% decreased to 4.20%(χ2=3.56,P<0.05), antimicrobial drug use to get effective control, antimicrobial unreasonable prescription proportion by 13.60% drop to 6.51%(χ2=28. 63,P<0.05), unreasonable double and triple therapy has been contained, triple drug prescriptions increased from 1.07%(before intervention)decrease 0.09%(u=12.36,P<0.05). The medication guidance on clinical pharmacists in the primary hospital outpatient, can effectively improve the basic level hospital outpatient clinical medication standard and rational drug use consciousness.
