Fulminant myocarditis suspected as acute myocardial infarction:a case report and literature review

Abstract:A 50-year-old female patient was admitted to the Emergency Department with a complaint of"chest discomfort for more than 9 hours".The emergency electrocardiogram indicated ST-segment elevation in the anterior wall,and high-sensitivity troponin I was significantly elevated.Acute anterior wall ST-segment elevation myocardial infarction was initially considered,but emergency coronary angiography showed no coronary stenosis or occlusion.Upon further inquiry,the patient had a history of transient syncope three days prior to admission and also presented with fever,peaking at 39.0℃,so fulminant myocarditis was considered.The patient's hemodynamics were unstable,and emergency treatment with endotracheal intubation and venoarterial extracorporeal membrane oxygenation(VA-ECMO)was initiated.After 8 days,the patient was successfully weaned off ECMO,followed by supportive treatment for symptoms,and the patient was discharged after recovery.
