Prognostic significance of prognostic nutritional index in patients with acute ischemic stroke and establishment of nomogram prediction model

Abstract:Objective To investigate the relationship between prognostic nutrition index(PNI)and the prognosis of patients with acute ischemic stroke,and to establish a nomograph prediction model.Methods A total of 217 patients with acute ischemic stroke diagnosed and treated in Shengjing Hospital of China Medical University from Jan 2016 to Jan 2020 were selected as the research subjects.There were 144 male cases and 73 female cases,with an age range of(67.1±8.1)years old,ranging from 40 to 80 years old.Collect the basic clinical characteristics,laboratory test results and treatment conditions of all patients.The influencing factors of prognosis in patients with acute ischemic stroke were analyzed through the Cox regression model,and a nomogram prediction model was constructed based on the influencing factors of prognosis.Results The best cut-off value of PNI was 46.32,the sensitivity was 63.6%,and the specificity was 82.1%.The 1-year,2-year and 3-year disease-free survival rates were 84.4%,78.2%and 67.9%respectively.According to the PNI cut-off value,it was divided into a low-value group and a high-value group.The 1-year,2-year and 3-year disease-free survival rates in the low-value PNI group were lower than those in the high-value group,and the difference was statistically significant(P<0.05).Multivariate Cox regression analysis showed that age>65 years old,history of hypertension,NIHSS score,early psychological care,and PNI were all independent influencing factors for the prognosis of patients with acute ischemic stroke.The C-index of the nomogram prediction model constructed based on the above independent influencing factors is 0.818.Conclusion PNI can be used as an important index to evaluate the prognosis of patients with acute ischemic stroke.The nomogram prediction model based on PNI can help clinicians to accurately evaluate the survival and prognosis of patients.
