Comparative Study on the Differential Diagnosis of Benign and Malignant Compression Fractures of the Vertebrae Using 18F-FDG PET/CT and 99mTc-MDP SPECT/CT
Abstract:Objective To explore the value of 2-[fluorine-18]-fluoro-2-deoxy-D-glucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) and 99mTc-methylene diphosphonate (99mTc-MDP) single-photon emission computed tomography/computed tomography (SPECT/CT) in differentiating benign and malignant vertebral compression fractures. Methods Forty-one patients with vertebral compression fractures diagnosed at the General Hospital of the Northern Theater Command, Chinese People's Liberation Army between February 2017 and December 2021 were selected. There were 23 males and 18 females, with a mean age of (47.0 ± 7.0) years (range, 24–71 years). All patients underwent 99mTc-MDP SPECT/CT bone imaging and 18F-FDG PET/CT. The benign or malignant nature of the fractures was determined by combining imaging findings, histopathological biopsy, and follow-up. The diagnostic efficacy of the visual analysis method, semi-quantitative analysis method of 18F-FDG PET/CT, and 99mTc-MDP SPECT/CT for the differentiation of benign and malignant vertebral compression fractures was analyzed. Results Finally, 18 patients were diagnosed with malignant fractures involving 19 lesions, and 23 patients with benign fractures involving 39 lesions. The diagnostic efficacy of the visual analysis method and semi-quantitative analysis method of 18F-FDG PET/CT was higher. Conclusion 18F-FDG PET/CT is an effective method for distinguishing benign and malignant vertebral compression fractures, with higher sensitivity and specificity compared to 99mTc-MDP SPECT/CT.
