Risk factors for secondary hypothyroidism after second surgery in patients with differentiated thyroid cancer
Abstract:Objective To explore the risk factors for secondary hypothyroidism (hypothyroidism) after second surgery in patients with differentiated thyroid cancer (DTC). Methods This study was a case-control study. A total of 120 patients with differentiated thyroid cancer who underwent second thyroid surgery at The First Hospital Affiliated to Hebei North University from January 2022 to January 2025 were selected, including 62 males and 58 females, with an average age of (45.7±11.3) years, ranging from 30 to 65 years. Clinical data of the two groups were collected, including age, gender, lesion site, lesion nature, type of first surgery, type of second surgery, interval between first and second surgery, 131I treatment dose before the second surgery (if received), thyroid hormone levels before the second surgery, history of alcohol consumption, smoking history, and family history of thyroid disease. Results Multivariate logistic regression analysis showed that age ≥45 years, female gender, bilateral lesions, malignant lesion nature, total thyroidectomy, and family history of thyroid disease were independent risk factors for secondary hypothyroidism after second surgery in patients with DTC (P<0.05). Conclusion Analyzing the risk factors for secondary hypothyroidism after second surgery in DTC patients can help take more proactive intervention measures, improve patients' quality of life, and provide scientific decision-making basis for postoperative management.
