Analysis of risk factors for postoperative massive gastrointestinal bleeding in patients with type A aortic dissection

Abstract:Objective To investigate the risk factors for postoperative massive gastrointestinal hemorrhage in type A aortic dissection(TAAD).Methods A total of 204 patients who underwent TAAD surgery in the General Hospital of Eastern Theater Command from January 2012 to December 2022 were selected,including 167 males and 37 females,with an age of(48.3±11.6)years old,ranging from 23 to 82 years old.Compare the preoperative,intraoperative and postoperative relevant indicators between patients who have experienced massive gastrointestinal bleeding and those who have not.Univariate analysis and multivariate Logistic regression analysis were used to explore the risk factors of postoperative massive gastrointestinal bleeding in TAAD patients.Results Logistic regression analysis indicated that preoperative involvement of the celiac trunk or mesenteric artery by the dissection and postoperative implementation of continuous renal replacement therapy were independent risk factors for massive gastrointestinal bleeding(P<0.05).Conclusion Dissection involving celiac trunk or mesenteric artery and postoperative CRRT are independent risk factors for postoperative massive gastrointestinal bleeding in TAAD patients.
