Analysis of Factors Influencing Recurrence After Surgery for Lumbar Disc Herniation
Abstract:Objective: To explore the influencing factors of recurrence after surgery for lumbar disc herniation (LDH). Methods: A total of 409 patients with LDH who underwent surgical treatment at Mianzhu People's Hospital from March 2019 to February 2023 were selected. There were 271 males and 138 females, with an average age of (56.21±5.73) years, ranging from 42 to 69 years. According to the recurrence status within one year after surgery, the patients were divided into non-recurrence group (n=358) and recurrence group (n=51). The age, body mass index, gender, smoking history, alcohol consumption history, underlying diseases, disease duration, pain level, disease type, degeneration degree, surgical method, underlying diseases, disease type, surgical segment, presence of Modic changes, preoperative sagittal range of motion (sROM), and intervertebral disc height were statistically analyzed. Multivariate logistic regression analysis was used to identify the factors influencing recurrence after surgery in LDH patients. Results: The overall recurrence rate within one year after surgery was 12.5% (51/409). Multivariate logistic regression analysis showed that age ≥60 years, presence of diabetes, high preoperative sROM, III-V grade degeneration, and presence of Modic changes were all risk factors for recurrence after surgery in LDH patients. Conclusion: The risk of recurrence after surgery in LDH patients is closely related to age, diabetes, preoperative sROM, degree of degeneration, and presence of Modic changes. For LDH patients aged ≥60 years, with diabetes, high preoperative sROM, III-V grade degeneration, and Modic changes, more attention should be paid postoperatively, and relevant health education should be provided to reduce the recurrence rate of LDH.
