Effect of comprehensive body temperature protection on the incidence of adverse reactions in elderly patients with giant rotator cuff tears after arthroscopic rotator cuff repair

Abstract:Objective To investigate the effect of comprehensive temperature protection on the incidence of adverse reactions in elderly patients undergoing arthroscopic rotator cuff repair for massive rotator cuff tears.Methods A total of 90 patients with massive rotator cuff tears who underwent arthroscopic rotator cuff repair at the General Hospital of Northern Theater Command from January 2016 to January 2022 were selected,including 38 males and 52 females,aged(69.11±2.92)years old,ranging from 65 to 75 years old.Based on perioperative temperature management,patients were divided into warming group and routine group.The routine group received standard intraoperative temperature control,while the warming group received comprehensive temperature protection.The two groups were compared regarding body temperature at different time points[baseline at general anesthesia induction(T0),30 minutes after surgery initiation(T1),60 minutes after surgery initiation(T2),90 minutes after surgery initiation(T3),120 minutes after surgery initiation(T4),and at the end of surgery(T5)],perioperative indicators[intraoperative fluid intake,duration of general anesthesia,perioperative blood loss,intraoperative saline usage,intraoperative urine output,time to general anesthesia recovery,extubation time,post-anesthesia care unit(PACU)stay duration],and adverse events(shivering,agitation,delirium).Results In the warming group,body temperature varied significantly across time points(P<0.05),with T0,T1 being lower than T3,T4,T5,the differences were statistically significant(P<0.05).In the routine group,body temperature also showed significant variation(P<0.05),with T0 and T1 higher than T2,T3,T4,T5,and T2 higher than T3,T4,T5,the differences were statistically significant(P<0.05).No significant difference was observed between the warming and routine groups at T0 and T1(P>0.05).However,the warming group had higher temperatures at T2,T3,T4,T5 compared to the routine group(P<0.05).No significant differences were found in intraoperative fluid intake,duration of general anesthesia,perioperative blood loss,intraoperative saline usage,or urine output between the two groups(P>0.05).The warming group had shorter general anesthesia recovery time,extubation time,and PACU stay duration,the differences were statistically significant(P<0.05).The incidence of shivering,agitation,and delirium was significantly lower in the warming group,the differences were statistically significant(P<0.05).Conclusion Comprehensive temperature protection can reduce the incidence of the incidence of adverse reactions in elderly patients undergoing arthroscopic rotator cuff repair for massive rotator cuff tears.
