Application of the Lateral Decubitus Direct Anterior Approach Combined with 3D Printing Technology in Total Hip Arthroplasty for Crowe I-III Developmental Dysplasia of the Hip
Abstract:Objective To explore the application value of the lateral decubitus direct anterior approach (LDAA) combined with 3D printing technology in total hip arthroplasty (THA) for Crowe I-III developmental dysplasia of the hip (DDH). Methods From January 2022 to June 2023, 21 patients with Crowe I-III DDH who underwent unilateral LDAA THA at the Affiliated Central Hospital of Shenyang Medical College were selected. There were 4 males and 17 females, with a mean age of (58.5±12.3) years (range 42-80 years). All patients underwent preoperative 3D model printing and three-dimensional surgical planning. Perioperative indicators, the matching rate of femoral and acetabular prosthesis models, early treatment outcomes [Harris hip score, visual analog scale (VAS), and lower limb length difference (LLD)], and complications during follow-up were recorded. Results The operation time was (92.6±21.4) minutes, the incision length was (10.1±1.2) cm, and the postoperative C-reactive protein level was 24.88 (17.77, 35.27) mg/L. The matching rate of the acetabular prosthesis with the planned model was 90.5% (19/21), and that of the femoral prosthesis was 85.7% (18/21). At 6 months postoperatively, the Harris hip score was higher than preoperatively; the VAS score and LLD level were lower than preoperatively, with statistically significant differences (P<0.05). During the follow-up period, no deep vein thrombosis, hip dislocation, periprosthetic fracture, infection, or limb numbness occurred. One patient experienced anterior thigh pain, which was resolved after analgesia and physical therapy. Conclusion The combination of LDAA approach and 3D printing technology for THA in adults with Crowe I-III DDH offers advantages such as minimal trauma, high surgical accuracy, and rapid functional recovery.
