Study on the Differential Diagnosis of Prostate Cancer Risk Level Using Apparent Diffusion Coefficient (ADC) from Magnetic Resonance Diffusion-Weighted Imaging (DWI), ADC Value Ratio, and Prostate-Specific Antigen (PSA)
Abstract:Objective: To investigate the diagnostic value of apparent diffusion coefficient (ADC) from magnetic resonance diffusion-weighted imaging (DWI), ADC value ratio, and prostate-specific antigen (PSA) in differentiating the risk level of prostate cancer. Methods: A total of 32 patients with non-metastatic prostate cancer, all male, aged (74.10±6.97) years (range 60–87 years), were selected from Chongqing Hongren Hospital and Chongqing Hechuan Hongren Hospital between November 2019 and July 2024. The ADC values of the cancer lesions, serum total prostate-specific antigen (tPSA), and free prostate-specific antigen (fPSA) were measured. The ADC value ratio and fPSA/tPSA were calculated for risk stratification of prostate cancer. The diagnostic performance of each indicator and their combination was evaluated using receiver operating characteristic (ROC) curves. Results: Among the 32 patients, 10 had intermediate-risk prostate cancer with 13 lesions, and 22 had high-risk prostate cancer with 51 lesions. The ADC values of intermediate-risk cancer lesions were higher than those of high-risk lesions, while the ADC value ratios were lower, with statistically significant differences (P < 0.05). There was no statistically significant difference in age or fPSA/tPSA between the two groups (P > 0.05). The tPSA and fPSA levels of intermediate-risk patients were lower than those of high-risk patients, with statistically significant differences (P < 0.05). ROC curve analysis showed that the combined diagnostic performance of tPSA, ADC value, and ADC value ratio was better. Conclusion: The quantitative combination of ADC values and ADC value ratios with serum tPSA has high value in distinguishing intermediate-risk from high-risk prostate cancer.
