Application value of oblique axial magnetic resonance of ankle in diagnosis of anterior talofibular ligament injury

Abstract:Objective To evaluate the diagnostic value of oblique axial magnetic resonance imaging(MRI)in the anterior talofibular ligament(ATFL)injury of the ankle joint.Methods A total of 73 patients,57 males and 16 females,with an average age of(24.14±1.75)years old,ranging from 22 to 28 years old,were selected for bilateral ankle magnetic resonance imaging(MRI)examination at the 970 Hospital of the Joint Logistics Support Force from January to May 2024.The positions of the patients examined were:neutral position in 21 cases and plantar flexion position in 52 cases.The patient underwent bilateral MRI scans of the ankle joint,and three types of axial positioning were performed in the conventional sagittal position:parallel to the tibiotalar articular surface(axis Ⅰ),Parallel to the long axis of talus(axis Ⅱ),Parallel to the median sagittal plane of the posterior border of the talar trochlea and the alignment of the anterior-superior border of the talus(axis Ⅲ).The full-length display of anterior talofibular ligament in axis Ⅰ,Ⅱ and Ⅲ was compared between neutral and plantar flexion patients.Results At neutral position,the ATFL movement was almost horizontal,and the scanning modes of axis Ⅰ and axisⅢ in neutral position were consistent with the ATFL movement,with no statistical significance(P>0.05).The scanning mode of axis Ⅱ was slightly different from that of ATFL,and the differences were statistically significant compared with that of axisⅠ and Ⅲ(P<0.05).When in plantar flexion position,ATFL walks downward;There was no significant difference in the ATFL display of axis Ⅱ and axial position Ⅲ in plantar flexion patients(P>0.05).There were statistically significant differences between axis Ⅰ,axis Ⅱ and axis Ⅲ(P<0.05).Conclusion For patients in neutral position or plantar flexion position,the oblique axis of the ankle joint is parallel to the median sagittal plane of the posterior border of the talar trochlea and the line between the anterior and upper border of the talar,and the observation of the anterior talofibular ligament is better.
