Effect of pre-rehabilitation on knee joint functional recovery after anterior cruciate ligament reconstruction

Abstract:Objective To investigate the effect of pre-rehabilitation on knee joint functional recovery after anterior cruciate ligament reconstruction(ACLR).Methods A total of 80 patients ACLR treat with anterior cruciate ligament rupture patients in General Hospital of Northern Theater Command from May 2020 to May 2023 were selected,including 59 males and 21 females,aged(29.46±5.99)years old,ranging from 19 to 44 years old.According to different rehabilitation plans,patients were allocated into the conventional rehabilitation group(n=40)and the pre-rehabilitation group(n=40).Pre-rehabilitation group received pre-rehabilitation before ACLR arthroscopy surgery combined with routine rehabilitation after surgery,and conventional rehabilitation group only received routine rehabilitation after ACLR arthroscopy surgery.Pain[visual analogue scale(VAS)score],balance(one leg standing test with eyes closed),knee function[international knee documentation committee(IKDC)score,knee injury and osteoarthritis outcome score(KOOS)score],quadriceps atrophy(difference of thigh circumference between affected side and healthy side),and cartilage injury of knee(Recht scale)were compared between two groups.Results It showed no significant difference in VAS scores between the two groups before surgery and 12 months after surgery(P>0.05).However,six months after surgery,the VAS score in the pre-rehabilitation group was significantly lower than that in the conventional rehabilitation group,with statistically significant(P<0.05).Prior to surgery,there was no notable difference in single-leg standing time,IKDC score,or thigh circumference discrepancy between the two groups(P>0.05).However,at 6 and 12 months post-operation,the pre-rehabilitation group demonstrated superior outcomes compared to the conventional rehabilitation group,with the differences being statistically significant(P<0.05).Prior to surgery,there was no significant difference in KOOS scores between the two groups(P>0.05).At six months post-surgery,the pre-rehabilitation group exhibited higher KOOS scores compared to the conventional rehabilitation group,with the difference being statistically significant(P<0.05).By 12 months after surgery,no significant differences were observed in pain and daily living function scores between the two groups(P>0.05).Symptoms,sport and recreation function,and knee-related quality of life scores were higher in the pre-rehabilitation group compared to the conventional rehabilitation group,with the differences being statistically significant(P<0.05).At 12 months post-surgery,a statistically significant difference in Recht grade was observed between the two groups(P<0.05).During the follow-up period,there was no recurrence of knee injury in both groups.Conclusion Pre-rehabilitation can improve balance,knee function,relieve quadriceps atrophy and knee cartilage injury in patients with anterior cruciate ligament reconstruction.
