Effect of threshold pressure load breathing training combined with extracorporeal diaphragmatic pacing on lung function in patients with tracheotomy after stroke

Abstract:Objective To investigate the intervention effect of threshold pressure load breathing training combined with extracorporeal diaphragmatic pacing on lung function in patients with tracheotomy after stroke.Methods A total of 120 patients with tracheotomy after stroke admitted to Cangzhou People's Hospital from January 2021 to January 2023 were selected and randomly divided into group A ( n=60 ) and group B ( n=60 ) .Group A underwent threshold pressure load breathing training,and group B was treated with extracorporeal diaphragmatic pacing on the basis of group A.The clinical efficacy,pulmonary function indexes[forced expiratory volume in the first second( FEV1),forced vital capacity( FVC)],Borg dyspnea scale score,diaphragm condition,and incidence of infection and success rate of tracheal extubation were compared between the two groups.The occurrence of adverse events during the treatment was observed in the two groups.Results The total effective rate of group B was 91.7%(55/60),which was obviously higher than that of group A[78.3%(47/60)],and the difference was statistically significant(P<0.05).After the treatment,FVC% and FEV1% in the two groups were higher than those before the treatment,and those in group B were higher than group A,the differences were statistically significant(P<0.05).After the treatment,Borg dyspnea scale score in the two groups was lower than that before the treatment,and that in group B was lower than group A,the difference was statistically significant( P<0.05) .After the treatment,the diaphragm thickness and diaphragm movement of the two groups were increased,and those of group B were higher than group A,the differences were statistically significant(P<0.05).The incidence of infection in group B was 3.3%(2/60),significantly lower than that in group A[16.7%( 10/60)],while success rate of tracheal extubation in group B was 61.7%( 37/60),significantly higher than that in group A[35.0%(21/60)],and the differences were statistically significant(P<0.05).During the treatment period,no serious adverse events occurred in both groups,and no adverse events such as asphyxia and coma occurred.Conclusion Threshold pressure load breathing training combined with extracorporeal diaphragmatic pacing can effectively improve lung function,regulate lung ventilation,reduce lung infection,improve patients'respiratory status,and promote disease recovery in patients with patients with tracheotomy after stroke.
