Clinical efficacy study of non-invasive positive pressure ventilation and ventilator oxygen therapy alternating mode in elderly patients with severe pneumonia complicated with respiratory failure

Abstract:Objective To investigate the clinical efficacy of non-invasive positive pressure ventilation and ventilator oxygen therapy alternating mode in elderly patients with severe pneumonia complicated with respiratory failure. Methods A total of 112 elderly patients with severe pneumonia combined with respiratory failure admitted from November 2022 to October 2023 were randomly divided into group A (n=53) and group B (n=59) by random number table method. Group A received continuous non-invasive positive pressure ventilation treatment,while group B received non-invasive positive pressure ventilation and ventilator oxygen therapy alternating mode. Heart rate,respiratory rate,mean arterial pressure(MAP),pulse oxygen saturation (SpO2). arterial blood gas analysis results,the patient comfort scores,and the incidence of facial skin injury,abdominal distension,difficulty of eating and sputum excretion were compared between the two groups before and 2 h,24 h and 48 h after treatment. Results The patient comfort scores in group B were significantly higher than those in group A at 24 h and 48 h after treatment,the difference was statistically significant(P<0.05). Heart rate and MAP in group B were higher than those in group A at 2 h after treatment,the difference was statistically significant(P<0.05). Partial pressure of carbon dioxide (PaCO2 )and Arterial oxygen saturation (SaO2) in group B were lower than those in group A,and pH was higher than that in group A at 2 h after treatment,with statistical significance(P<0.05). Partial pressure of oxygen (PaO2) in group A was higher than that in group B,and PaCO2 was lower than that in group B at 24 h after treatment,the difference was statistically significant(P<0.05). During treatment,the incidences of facial skin injury,abdominal distension,difficulty of eating and sputum excretion in group B were lower than those in group A,with statistical significance(P<0.05).Conclusion non-invasive positive pressure ventilation and ventilator oxygen therapy alternating mode can achieve the same oxygen therapy effect as continuous non-invasive positive pressure ventilation in elderly patients with severe pneumonia complicated with respiratory failure. Meanwhile,it can improve the comfort of patients and reduce the incidence of complications related to non-invasive positive pressure ventilation.
