Clinical efficacy and impact on pregnancy outcomes of different approaches of hysteromyomectomy

Abstract:Objective To investigate the clinical effects of different approaches of hysteromyomectomy on patients and its impact on pregnancy outcomes.Methods A total of 158 patientswith uterine fibroids who underwent hysteromyomectomy were selected as the research subjects.The patients were divided into the transabdominal group(n=71,abdominal hysteromyomectomy)and the laparoscopic group(n=87,hysteromyomectomy under laparoscopy)according to different surgical methods.Clinical indicators,postoperative visual analogue scale(VAS),incidence of postoperative complications and postoperative pregnancy outcomes of the two groups were recorded.Results The laparoscopic group had significantly shorter surgical time,postoperative exhaust time,hospitalization time,and postoperative antibiotic application time compared to the transabdominal group,with less intraoperative bleeding compared to the transabdominal group,and the differences were statistically significant(P<0.05).The incidence of complications in the laparoscopic group was 3.4%(3/87),significantly lower than that in the transabdominal group[12.7%(9/71)],the difference between the two groups was statistically significant(P<0.05).Theday of the surgery,1 day after the surgery,the VAS scores of patients in the laparoscopic group were significantly lower than those in the transabdominal group,with a statistically significant difference(P<0.05).After 3 days of surgery,there was no statistically significant difference in VAS scores between the two groups of patients(P>0.05).After surgery,there was no statistically significant difference in pregnancy rate,cesarean section rate,and abortion rate between the transabdominal group and the laparoscopic group(P>0.05).Conclusion Compared with transabdominal myomectomy,laparoscopic myomectomy has better clinical efficacy and has no significant impact on postoperative pregnancy outcomes.
