Blast-induced auditory organ injury:pathogenesis,diagnosis and treatment

Abstract:Blast-induced auditory organ injury refers to the complex damage to the external/middle ear,cochlea,and central auditory pathways from intense explosion shockwaves or high-intensity impulse noise.This multi-system trauma manifests clinically as conductive,sensorineural,or mixed hearing loss,frequently accompanied by tin-nitus and vertigo.Conductive hearing loss could often be improved through surgical intervention.Sensorineural hear-ing loss caused by blast should be treated as early as possible,and drugs that improve microcirculation in the inner ear and promote cell metabolism can be given to improve hearing.This manuscript delineates recent mechanistic in-sights and evaluates evolving interventions,synthesizing preclinical models,clinical studies,and preventive strategies to advance evidence-based management of blast-related auditory organ injuries for otologists and hearing specialists.
