Diagnostic accuracy of solid organ injuries in blunt abdominal trauma by contrast-enhanced ultrasound:a meta-analysis

Abstract:Objective To evaluate the diagnostic accuracy of solid organ injuries by contrast-enhanced ul-trasound(CEUS)in blunt abdominal trauma(BAT).Methods Chinese and English databases included PubMed,Web of Science,ProQuest,ScienceDirect,Ovid,Springer,CNKI,Wanfang,and VIP were searched for articles on the diagnostic accuracy of solid organ injuries in BAT by CEUS.The QUADAS-2 tool was used to evaluate the risk bias of the included articles.Meta-DiSc(version 1.4)was used for meta-analysis of sensitivity,specificity,and diagnostic odds ratio.Results A total of 13 articles with 14 studies were included in this meta-analysis for diagnostic accura-cy of solid organ injuries in BAT by CEUS.The random effect model showed a pooled sensitivity of 0.93(95%CI:0.90-0.95),a pooled specificity of 0.97(95%CI:0.95-0.98),a pooled diagnostic odds ratio of 219.14(95%CI:100.83-476.26),and an area under the summary receiver operating characteristics curve(AUC)of 0.976(standard error<0.001).The results of subgroup analysis showed that a pooled sensitivity of CEUS in the diagnosis of hepatic blunt trauma was 0.89(95%CI:0.80-0.95),a pooled specificity was 1.00(95%CI:0.98-1.00),and a pooled di-agnostic odds ratio was 482.96(95%CI:70.64-3302.03).A pooled sensitivity of CEUS in the diagnosis of spleen blunt trauma was 0.92(95%CI:0.87-0.95),a pooled specificity was 0.98(95%CI:0.96-0.99),and a pooled di-agnostic odds ratio was 657.74(95%CI:75.60-5722.22).Conclusion CEUS shows fair diagnostic accuracy for solid organ injuries in BAT and has a high clinical diagnostic value.
