Short-term efficacy of arthroscopic triple-procedure surgeries in the treatment of recurrent shoulder dislocation caused by military training

Abstract:Objective To evaluate the clinical efficacy of arthroscopic triple-procedure surgeries,respec-tively anterior glenoid labrum and capsule repair,subscapularis augmentation,and infraspinatus/capsular interval grafting,for the treatment of recurrent shoulder dislocation caused by military training.Methods A retrospective a-nalysis was conducted on 32 male soldiers with recurrent shoulder dislocation treated at the Department of Orthope-dics,General Hospital of Northern Theater Command between Jan.2022 and Feb.2023.All patients sustained injuries during training activities of throwing,seizing,obstacle race,etc.and had experienced≥3 times of disloca-tions.The mean age was 25.8 years(range:23-41 years),with 18 left and 14 right shoulders affected.The glenoid bone loss was 15.43%±2.45%,and dislocation frequencies were 6.3±2.4.All patients underwent the abovemen-tioned triple-procedure surgeries under arthroscope with a minimum follow-up of 1 year.The VAS score for pain,Constant-Murley score,American Shoulder and Elbow Surgeons(ASES)score,and Rowe Score for functional recov-ery were assessed before and 6 and 12 months after surgery,as well as the incidence of recurrent dislocations.Results All cases were followed up for 14 to 21 months,mean 17.3 months.At the three time points of before sur-gery,and 6 and 12 months after surgery,the affected shoulder showed VAS scores of 4(3,4),2(1,2)and 1(0,1),Constant-Murley scores of 55.3±4.5,75.1±8.0,and 85.0±3.7,ASES scores of 58(61,67),78(76,82),and 85(80,87),and Rowe scores of 44.8±3.6,72.4±8.8 and 80.3±6.4,all revealing significant differences between be-fore surgery and postoperative 6 or 12 months and between postoperative 6 and 12 months(all P<0.05).One case developed recurrent dislocation at follow-up,with no recurrence of dislocation within 10 months after revision surgery.Conclusion The arthroscopic triple-procedure surgeries for recurrent shoulder dislocation caused by military train-ing demonstrate largely reduced pain and significantly improved functional recovery with few recurrent cases,thus de-serving wide clinical application.
