An on-site real-time monitoring system in remote assistance of medical sergeants in conducting closed thoracic drainage

Abstract:Objective To investigate the effect of an on-site real-time monitoring(ORM)system based on augmented reality(AR)in remotely assisting medical sergeants with closed thoracic drainage(CTR)training.Methods This prospective study was conducted to analyze the training effects on 90 medical sergeants(75 males and 15 females,aged 21-24 years,mean 21.9 years)who received CTR training in the 909th Hospital in May 2020.The medical sergeants were randomly divided into ORM group(n=30,the trainees wore AR glasses and were remote-ly guided by experts to perform CTR with the assistance of an ORM system),independent group(n=30,the trainees conducted CTR without any assistance),and field guidance group(n=30,the trainees were guided by on-site experts to conduct CTR).Preoperative preparation,completion of key surgical steps,concept of love hurts,simplicity of op-eration,rationality of instrument use,fluency of operation,overall performance of operation,and operation time were scored based on the resident operation evaluation scale and compared among groups.Results The scores for ration-ality of instrument use showed no statistical difference among the three groups(P>0.05).The scores in the ORM group were 5(4,5)for preoperative preparation,5(4,5)for completion of key surgical steps,4(3,5)for concept of love hurts,5(4,5)for simplicity of operation,4(3,5)for fluency of operation and 5(4,5)for overall perform-ance of operation,all much higher than those in the independent group[4(3,5),3(3,4),3(3,4),4(3,4),3(3,4),and 4(3,5)(all P<0.05)],but not statically significant with the field guidance group[5(4,5),5(4,5),4(4,5),4(4,5),5(4,5)and 5(4,5)(all P<0.05)].The operation time(min)was 9.2±0.6 for the ORM group,much shorter than that for the independent group(10.2±1.2,P<0.05),but similar to the field guid-ance group(9.1±0.3,P>0.05).Conclusion ORM system is feasible and effective to remotely assist CTR con-duction,which improves the operation-related scores and first-line medical service ability.
