Osteoarticular infection by Aspergillus mycoses

Abstract:Osteoarticular infection by Aspergillus mycoses is chronic and easily misdiagnosed.Its diag-nosis and management are challenging and often require surgical intervention and long-period antifungal therapy.This review analyzes the epidemiology,clinical manifestation,diagnostic and therapeutic approaches,and outcomes of osteomyelitis and septic arthritis caused by these organisms.Most bone infections after trauma are related to Asper-gillus mycoses,especially for individuals with normal immune function.Diagnosis of fungal arthritis,because of the latency,slow progression and insignificant clinical characteristics,is quite challenging with fungi culture being the golden standard.Regarding the antifungal treatment,voriconazole can be the first choice.Recurrent infection mostly follows premature discontinuation of therapy.With timely diagnosis and standardized treatment,about 80%of pa-tients can obtain good outcomes.
