Clinical efficacy of remnant preservation and reconstruction with autologous semitendinosus for military training-related Sherman types Ⅱ and Ⅲ anterior cruciate ligament injury

Abstract:Objective To explore the clinical efficacy of autologous semitendinosus preservation and recon-struction of the anterior cruciate ligament(ACL)after Sherman types Ⅱ and Ⅲ ACL injuries due to military train-ing.Methods This retrospective analysis was conducted on 23 patients with partial ACL injuries admitted to our department from Jun.2022 to Jan.2023,with 22 males and 1 female aged 21-39(mean 28.5)years.The injury duration ranged 2-10(mean 2.9)months.All the cases had surgical indications and underwent ACL remnant pres-ervation and reconstruction with autologous semitendinosus.VAS,limb symmetry index(LSI),Lysholm score of the knee function,tibial anterior displacement distance,and surgery-related adverse reactions were evaluated before and 6 and 12 months after surgery.Results All cases were followed up for 12-18 months,with an average of 13.5 months.VAS,LSI,Lysholm score,and tibial anterior displacement distance showed significant improvement com-pared to those before surgery(data presented as preoperative,6 months and 12 months in sequence,i.e.,VAS:3.6±1.2,1.5±0.5,0.7±0.6;LSI:58.6%±6.7%,80.0%±8.2%,86.2%±7.5%;Lysholm score:65.7±10.8,80.2±8.5,86.3±7.6,tibial anterior displacement distance:11.3 mm±1.7 mm,3.9 mm±1.0 mm,4.1 mm±1.3 mm),all P<0.05 compared between preoperative and postoperative 6 and 12 months.No surgical technique re-duced adverse effect was reported.Conclusion Remnant preservation and reconstruction with autologous semiten-dinosus revealed satisfactory clinical efficacy for Sherman types Ⅱ and Ⅲ ACL injuries.The joint function and sta-bility were significantly improved at one year after surgery.
