Short-term effects of proximal femur bionic nail for intertrochanteric femur fractures with lateral wall rupture in the elderly

Abstract:Objective To evaluate the short-term therapeutic effects of proximal femur bionic nail(PFBN)in the treatment of intertrochanteric femur fractures with rupture of the lateral wall in the elderly.Methods A ret-rospective analysis was performed on 13 patients who underwent PFBN fixation for intertrochanteric femur fractures with lateral wall ruptures between Jan.2022 and Jun.2023.There were 3 males and 10 females with an average age of 76.9 years(range,69-83 years).The fractures were caused by road traffic accidents in 3 cases and falls in 10 cases.According to the AO/OTA classification,there were 4 cases of type 31-A3.1,6 of type 31-A3.2 and 3 of type 31-A3.3.All patients were treated by closed reduction and PFBN internal fixation.The operation time,intraop-erative blood loss,fracture healing time,incidence of complications,and Harris hip scores at the last follow-up were recorded to analyze the clinical effect.Results The operation time was 25-70(mean,43.7)min,intraoperative blood loss was 100-300(mean 151.3)mL.All the patients were followed up for 4-16 months,with an average of 10.5 months.The fracture reduction quality was rated as excellent in 8 cases,good in 4,and poor in the rest 1 by Chang's criteria.Bone union was obtained in 8-18(mean 12.5)weeks.After the operation,one case had mild pain on the hip's lateral side.None of the patients reported complications of nonunion,cutting out or breakage of in-ternal fixation devices,or avascular necrosis of the femoral head.By the Harris scores at the last follow-up,7 cases were excellent,5 good and 1 fair,with the excellent and good rate being 92.3%.Conclusion PFBN in the treat-ment of intertrochanteric femur fractures with rupture of the lateral wall has few postoperative complications and good short-term therapeutic effects.
