Effect of a remote wire-controlled device for bone cement injection in percutaneous vertebroplasty

Abstract:Objective To explore and summarize the application effect of a bone cement injection equip-ment controlled by a remote wire in percutaneous vertebroplasty(PVP).Methods This retrospective analysis was conducted on 52 patients with single-segment osteoporotic thoracolumbar compression fractures admitted to our de-partment from Jun.2018 to Dec.2021,including 18 males and 34 females aged 59-84(mean 67.6)years.Among them,32 cases were injured due to falls,6 to moving heavy objects,7 to smashing injuries,and 7 did not have obvi-ous causes.According to different methods of bone cement injection,patients were divided into two groups.The con-trol group consisted of 29 cases with the bone cement injected by using a manually controlled spiral thruster,while the observation group consisted of 23 cases with the bone cement injected by using a remote wire-controlled device(fix the propeller at the front end of the control cabin and use the wire-controlled operation panel to control the screw to inject bone cement after starting).The VAS before and 3 d after PVP,frequency of intraoperative C-arm expo-sure,bone cement distribution,and bone cement injection time and volume were analyzed.Results Both groups showed significantly reduced VAS at 3 d after PVP,compared with that before surgery(both P<0.05).The differ-ence between the two groups was insignificant for both VAS and incidence of bone cement leakage(P>0.05);but the observation group had a much shorter cement injection time(s,187.0±14.1 vs.239.7±23.7),smaller bone ce-ment volume(mL,4.50±0.64 vs.6.21±1.01),fewer times of intraoperative C-arm exposures(8.4±1.6 vs.11.4±1.8),and better bone cement distribution(82.96%±6.29%vs.75.24%±4.75%)(all P<0.05).None of the pa-tients experienced serious complications.Conclusion Bone cement injection by this remote wire-controlled device is satisfactory.Compared with the traditional manual control method,it reduces radiation exposure to both patients and medical staff and shortens the injection time,and thus is worth wide application.
