The effect of vertebroplasty combined with Schanz screw in the treatment of senile thoracolumbar vertebral compression fracture

Abstract:Objective To investigate the clinical efficacy of vertebroplasty(PVP)combined with Schanz screw in the treatment of thoracolumbar vertebral compression fractures in elderly patients,and analyze the difference in efficacy and safety between PVP and kyphoplasty(PKP).Methods A total of 105 patients with osteoporotic vertebral compression fracture in our hospital from Jan.2016 Mar.2017 were divided into PVP screws combined with Schanz group and PKP group according to treatment method.Difference in age,sex,body mass index(BMI), bone mineral density(BMD)of patients in the two groups was not statistically significant.The operation time,bone cement,bone cement leakage,preoperative and postoperative vertebral kyphosis angle(Cobb angle),and the differ-ence between the preoperative and postoperative VAS score of the two groups were recorded and analyzed with SPSS20.0.Results Compared with preoperative score,postoperative VAS score of both groups decreased signifi-cantly,and the difference was statistically significant.Difference between preoperative and postoperative vertebral kyphosis angle(Cobb angle)in the PVP and Schanz screw group was 11.23 ±2.34,and was 4.12 ±0.98 in the PKP group.The two groups had significantly statistical difference.Two groups of patients had no significant differ-ence in the volume of bone cement and bone cement leakage.The operation time of PVP combined with Schanz screw group was(61 ±12)minutes and was(42 ±11)minutes in the PKP group,and the difference was statistical-ly significant.Conclusion The two surgical methods both have good short-term clinical effects in relieving pain. In the correction of kyphosis,PVP combined with Schanz screws is better than PKP,but the operation time is longer.
