Effect of vertebral pedicle bone graft of injured vertebral arch combined with pedicle screw fixation on the nerve function in patients with severe thoracolumbar burst fracture

Abstract:Objective To discuss the effect of vertebral pedicle bone graft of injured vertebral arch com-bined with pedicle screw fixation on the nerve function in patients with severe thoracolumbar burst fracture(STBF). Methods A total of 100 patients with STBF from Dec.2013 to Dec.2016 in our hospital were selected.According to the random distribution,all patients were divided into across injured vertebral group and via injured vertebral group,with 50 cases in each group.The across injured vertebral group was given vertebral pedicle bone graft across injured vertebral arch combined with pedicle screw fixation treatment,and the injured vertebral group was given ver-tebral pedicle bone graft of injured vertebral arch combined with pedicle screw fixation treatment.The pre -and postoperative vertebral front height,Cobb angle,compression area,nerve function and complications were compared between the two groups.Results The postoperative vertebral front height((52.57 ±5.51)mm),nerve function excellence rate(88%)of injured vertebral group were significantly higher than those of across injured vertebral group((45.29 ±5.32)mm,64%).The postoperative convex Cobb angle((9.82 ±1.37)°),compression area ((21.52 ±2.24)mm2)of injured vertebral group were significantly lower than those of across injured vertebral group((6.65 ±1.18)°,(17.81 ±2.01)mm2),and the differences were statistically significant(P<0.05).The complication rates of across injured vertebral group and injured vertebral group were basically the same,and there was no statistically significant difference(P>0.05).Conclusions Compared with across injured vertebra treat-ment,vertebral pedicle bone graft of injured vertebral arch combined with pedicle screw fixation can effectively im -prove the vertebral height and compression state in patients with STBF.It is helpful to improve the recovery of neu-ral function in patients,and it has good security and is worthy for further clinical promotion.
