The preoperative incidence of deep vein thrombosis and its correlation with postoperative DVT in patients undergoing elective surgery for femoral neck fractures

Abstract:Objective To study the prevalence of preoperative deep vein thrombosis(DVT)in patients with acute hip fractures and the related risk factors,and to explore the relationship between preoperative and postop-erative DVT.Methods Totally 108 femoral neck fracture patients undergoing delayed operation(including total hip replacement or artificial femoral head replacement)in our hospital from 2011 to 2016 were retrospectively ana-lyzed.Clinical data were collected and preoperative and postoperative conditions were examined by venography to di -agnose DVT.Preoperative VTE patients were treated by thrombolytic and anticoagulation(low molecular weight heparin or rivaroxaban).After operation,suggested anticoagulation therapy including mechanical prevention(such as early activity,elastic socks,venous foot pump and intermittent pneumatic compression)and drug prevention (such as low molecular heparin and rivaroxaban)were used to prevent thrombus.DVT was diagnosed by repeated intravenous contrast.Univariate analysis and multivariate logistic regression analysis were used to determine the risk factors for preoperative DVT.Results Thirty-five patients(32.4%)had DVT during the preoperative DVT.De-layed operation time and D-dimer were independent risk factors for preoperative DVT.Moreover,24 patients (64.9%)had DVT and preoperative DVT occurred in the same position.Conclusion This study shows that the incidence of DVT in patients with femoral neck fracture surgery is high,and DVT patients are more related to preop-erative DVT.
