Clinical experience of emergency laparoscopic splenectomy for treating splenic trauma of grade Ⅲ and above

Abstract:Objective To discuss the indication and clinical effects of emergency laparoscopic splenectomy for treating splenic trauma of grade Ⅲ and above.Methods Clinical data of 32 traumatic splenic rupture patients was collected and analyzed ,who underwent emergency laparoscopic splenectomy from Aug .2011 to Jun..2016 in our hospital .The following data including operative time ,intraoperative blood loss volume ,average hospital stay and postoperative complications was analyzed .All patients were traumatic spleen rupture without severe combined inju-ries.There were 19 cases of grade Ⅲ(59.4%),10 cases of grade Ⅳ(31.2%),and 3 cases of grade Ⅴ(9.4%). Results Twenty-seven cases achieved successful lapatoscopic slpenectomy and 5 cases were transferred to laparoto-my.There were 24 cases of intraoperative autologous blood transfusion (75.0%),and 31 cases of blood transfusion (96.9%) .The average peri-operative bleeding was (1682 ±582)mL,average operative time was (145.5 ±62.5) min,the average postoperative fasting time was (2.9 ±1.9)d,and the average postoperative hospital stay was (8.6 ± 5.4)d.The overall postoperative morbidity rate was 12.5%(4/30),including 1 case of pancreatic leakage ,1 case of pleural effusion,and 2 cases of infections.There was no death case and serious complications .Conclusion Emer-gency laparoscopic splenectomy for treating splenic trauma of grade Ⅲand above has advantages such as mini-inva-sion,fast recovery,few complications and short hospital stay .Minimally invasive operations are safe and feasible ,and deserve further clinical applications .
