Surgical method and effect of free implantation of thoracolumbar flap for repairing soft tissue defect of foot and ankle

Abstract:Objective To explore the surgical method and effect of free implantation of thoracolumbar flap for repairing soft tissue defect of foot and ankle under the microsurgical technique .Methods Clinical data of 80 cases of ankle soft tissue defects was retrospectively analyzed , who were admitted and treated inPeople 's Hospital of Sanya from May 2011 to Dec.2015.There were 57 males and 23 females,with a mean age of 39.4 years(range,18 to 67 years).Soft tissue defect sites in cluded foot back in 14 cases, ankle in 40 cases(15 cases in medial malleo-lus, 20 cases in lateral malleolus, and 5 cases in ventral condyle ankle ), and heel in 26 cases.Traumatic defect area ranged from 8.2cm ×4.9cm to 11.6cm ×11.7 cm.Thirty cases were combined with wound infection .All pa-tients underwent free implantation of thoracolumbar flap to repair soft tissue defect of foot and ankle .The survival rate of the flap,the function of the ankle and the sensory function of the flap were observed .Results All the 80 patients were successfully completed the surgical treatment .Flaps completely survived and the wound reached the I level of healing standard in 72 patients,and cyanoderma and low skin temperature ( <26 ℃) were found in 8 pa-tients after flap transplantation ,of whom 7 cases had flaps survived by the timely treatment of venous crisis , and 1 case developed extensive flap necrosis due to transplantation failure .The mean follow-up time was 7.8 months (range,6 to 9 months).Among the 79 patients with survived flaps ,76 patients were obtained the follow-up data(3 of them were unable to be followed up for various reasons ) .Evaluation of sensory function after 3 months indicated 11 cases (14.47%) in S1 grade,18 cases (23.69%) in S2 grade, 40 cases (52.63%) in grade S3 and 7 cases (9.21%) in grade S4.AOFAS ankle and hind foot function score showed excellent in 23 cases (30.26%), good in 37 cases (48.68%), fair in 11 cases(14.47%) and poor in 5 cases(6.58%).Conclusion Under the micro-surgical technique ,the free implantation of thoracolumbar flap for repairing soft tissue defect of foot and ankle can a -chieve high flap survival rate and good ankle joint function recovery .
