Diagnosis and treatment of intestinal rupture in multiple trauma patients

Abstract:Objective To discuss the diagnosis and treatment of intestinal rupture in multiple trauma pa-tients. Methods Thirty-seven multiple trauma patients with intestinal rupture admitted in Trauma Surgical Depart-ment of Tongji Hospital from Jan. 2012 to Dec. 2016 were analyzed retrospectively. In this study,19 patients suf-fered from traffic injuries,9 from falling off high places,1 by trumpling,and 1 patient was injured in an explosion. Results The rate of missed diagnosis was 29. 7%(20/37) and not related to the location of the bowel. The rate of missed diagnosis in severe multiple trauma patients(ISS>16) was significantly higher than that in less severe multi-ple trauma patients(ISS≤16). All patients received operation and 4 patients died. The mortality rate was 10. 8%. Conclution More severe the multiple injures are,higher the rate of missed diagnosis is. FAST,CT and laparoscopy are all valuable diagnositic tools in early diagnosis and can be given repeatedly. Traditional exploratory laparotomy is still preferred treatment for intestinal rupture.
