Application of "bridging method" in patients with closed pancreatic injury accompanied with pancreatic duct rupture in the anterior vertebral region

Abstract:Objective To explore the application value of "bridging method" in patients with closed pancreatic injury accompanied with pancreatic duct rupture in the anterior vertebral region.Methods A retrospective cohort study was carried out.The clinical data of 28 patients with pancreatic injury and pancreatic duct rupture in our hospital from Oct.2008 to Oct.2015 were collected.Thirteen patients treated before May 2012 with pancreatic and duodenal resection or pancreatic intestinal anastomosis were established as the conventional surgery group.Fifteen patients treated after May 2012 with"bridging method" were established as the "bridge" surgery group.Observation indexes included: (1) the operation time and surgery related hemorrhage;(2) the time of tube pulled out,ICU stay,hospital stay and postoperative complications.Results Two groups of patients were done surgery successfully.The operation time in the "bridge" surgery group and the conventional surgery group was (2.6±1.4) h and (5.8±3.9) h,respectively.Surgery related hemorrhage in the "bridge" surgery group and the conventional surgery group was (147.3±79.2) mL and (160.3±75.6) mL,respectively.The time of tube pulled out was (3.4±1.2) d and (4.2±1.3) d,ICU stay was (1.2±0.4) d and (2.5±1.1) d,the time of hospital stay was (8.3±3.3) d and (11.7±5.9) d,and there were statistically significant differences in the above indexes(P<0.05).The rate of postoperative pancreatic fistula in the "bridge" surgery group and conventional surgery group was 6.7% vs.7.3%,of abdominal infection was 13.3% vs.14.7%,of postoperative bleeding was 13.3% vs.15.3%,of pulmonary infection was 33.3% vs.30.7%,of postoperative diarrhea was 6.7% vs.8.3% and of diabetes was 5.7% vs.6.7%,and the differences were not statistically significant (P>0.05).Conclusion The "bridging method" does not increase the incidence of postoperative pancreatic fistula,abdominal infection,hemorrhage,pulmonary infection and incidence of pancreatic diarrhea,diabetes,hypothyroidism during the follow-up in patients of closed anteriorvertebral pancreatic injury combined with pancreatic duct fracture.It has advantages in shortening operation time,reducing intraoperative bleeding,shortening the tube pulled out time,ICU stay time and hospital stay time.
