Exploration of the relationship between postoperative bone cement volume fraction and adjacent vertebral fractures after unilateral vertebral percutaneous kyphoplasty

Abstract:Objective To investigate the relationship between bone cement volume fraction and the adjacent vertebral fractures after unilateral vertebral percutaneous kyphoplasty. Methods A total of 106 cases of single osteoporotic vertebral compression fractures who were admitted to our hospital from Sep.2014 to Mar.2016 and received unilateral percutaneous vertebral kyphosis angioplasty were retrospectively analyzed.They were divided into AVF group and non-AVF group according to the vertebral body new fractures, with 53 cases in each group.Two groups of patients were compared with bone cement volume fraction, and bone cement permeability, Cobb angle,VAS score,ODI index,vertebral height and surgical conditions were compared before and after surgery. Results The operation time had no statistically significant difference(P>0.05);patients in the AVF group had more bone cement injection volume, higher bone cement permeability and bone cement volume fraction [(4.52±0.21)mL,(15.96±2.14)%,(30.25±4.12)%] than non-AVF group [(3.12 ± 0.31)mL,(12.78±3.02)%,(26.45±3.12)%], and the differences were significant (P<0.05);after surgery, VAS score and ODI score of two groups were significantly lower than before treatment, the differences were significant (P<0.05);before surgery, the difference of the anterior height,the central height and Cobb's angle between two groups of patients was not statistically significant (P>0.05);after surgery, the anterior height and central height of the two groups of patients were higher than before treatment,and the difference was significant (P<0.05);Cobb angles of the two groups were both lower than those before treatment, and the difference was significant (P<0.05). Conclusion The bone cement volume fraction can determine the amount of bone cement, which can get a good effect,and the AVF leakage rate of cement will increase as the volume fraction of cement increases.
