Analysis of risk factors for prolonged ICU time in multiple trauma patients

Abstract:Objective To explore the risk factors of intensive care unit (ICU) time delay (≥14d) in severe trauma patients.Methods A total of 834 patients with polytrauma were studied retrospectively,who were admitted to the and ultimately survived from Jan.2010 to Dec.2016. The 11 variables (including age,gender,underlying diseases,ISS,infection,pre-hospital care,surgery,type of injury,emergency room treatment time,coma,mechanical ventilation) that might affect or extend patients' ICU were analyzed and the key independent risk factors that could prolong ICU time (≥14d) were discovered.Results There were 96 patients whose ICU time was prolonged,accounting for 11.51%.The four variables including ISS,injury type,emergency room treatment time,and mechanical ventilation were the independent risk factors that lead to prolonged ICU time.Conclusion In the trauma treatment chain,we should pay high attention to severe patients with ISS ≥16 and high energy injury.Meanwhile,we should accelerate the intra-hospital processing speed,optimize the emergency procedures,simplify procedures for admission,perform the definitive treatment of severe trauma patients within the shortest time,and try to shorten the ICU time of severe trauma patients and improve their prognosis.
