Clinical effect of bone graft combined with oblique T-plate fixation in the treatment of elderly patients with unstable distal radius fractures

Abstract:Objective To observe the clinical effect of bone graft combined with oblique T-plate fixation in the treatment of unstable distal radius fractures in the elderly.Methods Eighty elderly patients with unstable distal radius fractures treated in our hospital from Jan.2011 to Jan.2015 were selected,who were randomly divided into study group and control group,with 40 cases in each group.The study group was treated with autogenous bone or allograft bone combined with oblique T-plate internal fixation,while the control group was treated with external fixation.The palmar tilt,ulnar deviation and radial height of the two groups were measured and compared before operation and at 6 months after the operation.Wrist mobility was compared at 6 months postoperatively at the same time.The wrist function was compared according to the Gartland-Werley wrist function score.The clinical effects and complications were compared.Results The 80 patients were followed up for 6-12 months (average,(9.3±3.2) months).There was no significant difference between the two groups in preoperative palmar tilt,ulnar deviation and radius height (P>0.05).At 6 months after operation,the palmar tilt,ulnar deviation and radial height were significantly different from those before the operation(P<0.05),and the change of the study group was obviously higher than the control group (P<0.05).The patients in the study group were significantly better than those in the control group in the aspects of wrist extension,palmar flexion,pronation and supination,and the differences were statistically significant (P<0.05).For B2 type,B3 type,and C1 type fractures,the Gartland-Werley wrist function score was lower than that of the control group,and the difference was statistically significant (P<0.05),but for C2 type and C3 type fractures,the two groups had no significant difference (P>0.05).In clinical curative effect,there were 30 cases of excellent,5 good,4 fair,and 1 poor in study group with the excellent and good rate of 87.5%,while there were 18 cases of excellent,10 good,7 fair,and 5 poor in the control group,with the excellent and good rate of 70%,and the difference was statistically significant between the two groups (P=0.043).With regard to the postoperative complications,the incidence of complications was 10.0% in the study group and 22.5% in the control group.Conclusion Bone graft combined with oblique type T-plate fixation in the treatment of unstable distal radius fractures in the elderly has obviously curative effect,can significantly improve the cure rate of patients,and greatly reduces the incidence of complications.It is worthy to be widely promoted in clinical application.
